This is a consent form for someone under 18 to take part in adventurous activities (caving/climbing /abseiling) with Learn to Cave. For young people to take part in activities we need the consent of the parent or guardian to confirm that they have acknowledged the potential risks involved in adventurous activities and all disclosed medical information is correct.
By completing this form you are giving permission for the young person you are responsible for, to take part in the planned adventurous activity.
Climbing, caving and abseiling, when experienced as part of a well organised guided group are safe activities. However, they can be physically demanding in a potentially challenging environment where all risk cannot be eliminated.
It is important that we know of any medical/personal information that might affect the young person’s ability to take part fully in the activity. As their parent or guardian, please can you fill in the following information about the young person taking part in the activity and return it to Learn to Cave. If anything is unclear or you have any concerns, please contact Learn to Cave.
