I understand that adventurous activities are inherently hazardous and cannot be completely risk free and can include obstacles of a substantial and physically testing nature. Accidents can happen without any contributory negligence from the staff and Learntocave will not be liable for personal injury not arising as a result of its own negligence.
Should the occasion arise, I give my consent to any emergency treatment necessary. I therefore authorise the group leader(s) to sign, on my behalf, any written form of consent required by the hospital authorities should medical treatment (a surgical operation or injection) be deemed necessary, provided that, in the opinion of the Doctor or Surgeon concerned, any delay in obtaining my signature could endanger my/my son’s/my daughter’s health or safety.
Please note that personal accident and loss of belongings are not insured but participants are covered by “Taylor Made Training” insurance in the event of negligence by one of the employees or agents of “Taylor Made Training”.

The information being collected on this form will only be used for the purpose of administration.
The data will not be disclosed to any external sources other than in an emergency without your written consent.
TERMS AND BOOKING CONDITIONS
1.1 No variation of these terms & conditions shall be applicable unless agreed in writing by Taylor Made Training Ltd T/A learn to cave before booking the relevant course.
1.2 A booking is only confirmed upon the receipt of an e-mail confirming the booking details and payment of a deposit.
CANCELLATION BY CLIENT
1 All cancellations must be made in writing.
2.2 Taylor Made Training Ltd T/A Learntocave once notified by letter or Email transmission of the cancellation would use his best endeavours to resell the course place, or confirm a new booking .
2.3 In the event that the course/ date is resold prior to the commencement of the course/caving day, the client will receive a full refund less 10% of the course fee to cover administration costs.
2.4 Refunds will only be made after the start date of the course has commenced.
2.5 Taylor Made Training will always endeavour to resell courses/caving days whenever possible
2.6 In the event that the course is not resold prior to the commencement of the course, the client will be charged the following cancellation fees: Charge Incurred Cancellation more than 30 days prior to the start of the course Loss of deposit Cancellation between 14-28 days prior to the start of the course 50% of total course cost Cancellation less than 14 days prior to the start of the course 100% total course cost
2.7 A ‘postponement’ will be treated as a cancellation and a re-booking.
CANCELLATION BY TAYLOR MADE TRAINING LTD T/A LEARNTOCAVE
3.1 Whilst every attempt is made to ensure that caving sessions actually run, Taylor Made Training Ltd T/A Learntocave will notify the client of cancellation as soon as practicable where we believe on reasonable grounds that cancellation is necessary due to dangerous and/or unsuitable conditions for the course /activity.
3.2 Taylor Made Training Ltd T/A Learn to cave shall notify the client of likely cancellation ten days prior to the start of the course and of actual cancellation not less than five days, where for whatever reason it is not practical or safe for the caving activity to take place
3.3 In the event of cancellation clients will be offered the choice of the following options: – a: Full refund of the course fee paid b: Another course on different dates.
3.4 Taylor Made Training reserve the right to remove a client from a course due to incapacity or inability to fulfil the demands or the course on grounds of safety. In such circumstances the client will not be entitled to a refund. Taylor Made Training Ltd T/A Learntocave will not be liable for any losses resulting from this decision.
3.5 In circumstances where cancellation may be necessary such as extreme weather conditions, flooding affecting caves and potholes, rivers rising, fire risk on moorland areas or access restrictions placed on the public such as foot and mouth in 2001. Personal insurance against cancellation is recommended.
LIABILITY AND RISK
4.1 All courses delivered by Taylor Made Training Ltd T/A Learntocave take place in areas that are not without inherent risk. Taylor Made Training, T/A Learn to cave make every effort to minimize risks to participants. Should any risk remain, your Instructor employed by Taylor Made Training T/A Learn to cave will then advise the safest way to proceed. All clients must follow recommendations made by their Instructor.
4.2 All risk cannot be removed entirely; therefore any clients booking with Taylor Made Training Ltd T/A Learntocave must accept these risks as part of their chosen course.
4.3 Taylor Made Training Ltd T/A Learntocave will only accept liability for injury to a client where negligence of an Instructor working for Taylor Made Training T/A learn to cave is proven.
4.4 Taylor Made Training T/A Learn to cave recommend all clients obtain their own insurance to cover any accident or injury that may occur.
